How It Works

Schedule Your Virtual Home Safety Assessment
Review the client agreement and click the button below to schedule your assessment at a time that’s convenient for you. Each assessment is 55-minutes and you or your designated caregiver will receive a comprehensive written report. Please come to the virtual appointment with a well-lit, camera-ready home and appearance.

Meet with Our Expert Haven Specialist
Our expert haven specialist will guide you through a virtual walk-through of your home, evaluating key areas for safety, mobility, and dementia-friendly features. Please come to the virtual appointment with a camera-ready appearance and home. You must be able to walk through your home with a mobile camera connected via our online meeting platform.

Receive Your Personalized Safe Haven Home Assessment Report
After the assessment, we’ll send you a detailed report with recommended modifications.

Payments

You may submit your payment using a personal credit or debit card. No refunds if you cancel less than 48 hours before your appointment or miss your appointment time. Medicare provides coverage for home safety assessments with a referral from your primary care physician. Request Safe Haven Assessments. You may also submit your assessment report and request reimbursement from your insurance provider using the HCPCS code of T1028 for home environment assessment.
